Difficulties creating relationships
Creating has a lot more Google hits than forming, as I suspected. I don't see any real difference but languages have a logic of their own.

In Education services, youth services and so on, "relationship difficulties" is what's often recognised as a condition for some children on SEN statement. In more general terms, you could also say "children with social difficulties"
