I am assuming that the drawing referred to is a drawing of a building (or part thereof) that has to be physically measured

My guess...we would need more context. Sometimes, drawings are deformed (eg. the vertical scale is greater than the horizontal one)...that would be a case when it's not "true to size"

I don't think the context could help very much. It's the explanation of an exercise (Descriptive Geometry). Here one of the examples (there is more than one example in the text):
"sarà sufficiente riportare le misure (o dimensioni, n.d.t.) al vero sulla L.T."
I hope it could help :)
In other words, the dimensions of something on a drawing, for which the dimensions are given thereon, are always the real dimensions. For that reason it would be inappropriate, and confusing, to say "real dimensions".
