Hi Joseph I'm not sure what you mean by "apparently glossed as", but if you Google these two words, you'll find that Caiazzo is a town in the province of Caserta. Does that answer your question?

Origine del cognome Caiazzo
Deriva dal toponimo Caiazzo (Caserta), legato al tardo latino Caiatia, da cala, "rocca/luogo elevato".
Il cognome Caiazzo è decisamente campano, del casertano in particolare.
Caiazza è maggiormente presente nel napoletano.
Caiazzi, quasi unico, è romano.
Cajazzo è praticamente unico.
