Context: "Io, quando sento una nota stonata o un canto sgraziato ho i *brividi sulla pelle*".
I just want to make sure that I get the right tone - Would "makes my skin crawl" be an accurate equivalent as I cannot find this in my dictionaries.

yes, definitely, makes my skin crawl has a negative connotation.

very accurate - in this case, it is clearly an unpleasant experience the author is referring to, so the expression you suggest seems quite suitable.

This is what happens to me when I hear a note sung out of tune (like by myself!)

so "I cringe when I hear a note sung out of tune or a song sung badly" or "A note....makes me cringe"

Why not "i've got creeps..."

Personally i'd say simply i've got shivers, but "brividi sulla pelle" also let me think about "pelle d'oca" which in english should be creeps!!!
