Strictly speaking that's true, but in the hundreds of these I've translated the two terms in Italian are used more or less interchangeably. As Lionel specified "che firma il paziente", I'd say they mean the form here.

A patient signs a declaration of informed consent in order to acknowledge that they are aware of the consequences of accepting the treatment.
