With the help of the answers that have already been proposed, I suggest this alternative. The word 'unico' can indeed be translated as 'single' or 'unique' (between the two, I would choose 'single' in this case) but given the nice description of this STU that Juliet has provided, I think that 'unified therapy record' may come closer to expressing something that integrates 'tutte le informazioni'.

: this is a difficult term to translate - "unified" crossed my mind too - I'm not sure what the concept would be called in English in a succinct way - all the patient's treatment details in a single document
