reversed Z-plasty Spiegazione: Z-plasty is a type of plastic surgery used to revise scars and help them to appear less noticeable. The surgery cannot remove scars altogether and doesn’t attempt to. Instead, it works to camouflage them by changing their positioning. When a person undergoes Z-plasty, his surgeon changes the position of his scar to more closely match the normal lines of his skin, making the scarred area stands out less. This type of surgery is considered particularly helpful for dealing with facial scarring, as the normal lines, wrinkles, and creases of the face provide easy camouflage for scar tissue. Often, scars form and heal outside of the skin’s natural lines, which makes them appear very prominent. When a plastic surgeon performs a Z-plasty, he works to reposition scar tissue and cause it to heal along the skin’s normal lines. To perform this surgery, a surgeon creates incisions around the scar, forming skin flaps. The plastic surgeon then lifts the flaps of skin and positions them over the scar line. This changes the tension between the scar tissue and the patient’s skin. https://www.wisegeek.com/what-is-z-plasty.htm Z-plasty is a versatile plastic surgery technique that is used to improve the functional and cosmetic appearance of scars. It can elongate a contracted scar or rotate the scar tension line. The middle line of the Z-shaped incision (the central element) is made along the line of greatest tension or contraction, and triangular flaps are raised on opposite sides of the two ends and then transposed. The length and angle of each flap are usually the same to avoid mismatched flaps that may be difficult to close. Some possible complications of Z-plasty include flap necrosis, haematoma (blood clot) formation under the flaps, wound infection, trapdoor effect and sloughing (necrosis) of the flap caused by wound tension and inadequate blood supply. https://en.wikipedia.org/wiki/Z-plasty Summary Achilles tendon repair, and particularly reconstruction of neglected tendon ruptures, is associated with a high incidence of wound complications. Insufficient circumferential skin laxity because of soft tissue swelling and loss of the Achilles tendon retinaculum may lead to difficulties in the closure of longitudinal incisions, despite there being adequate skin laxity in a vertical direction. We used a reversed Z‐plasty skin incision in seven patients undergoing Achilles tendon reconstruction with FHL or FDL tendon transfer. The incision is posteromedial incorporating the reversed Z‐plasty at the malleolar level. Full thickness flaps are elevated. At wound closure, the flaps are reversed utilizing the longitudinal skin laxity to increase the skin available in a circumferential direction. All wounds closed without tension and healed without complication, with a cosmetically acceptable result. The simplicity of the technique and the ease with which wound closure is achieved lead us to recommend it. https://onlinelibrary.wiley.com/doi/abs/10.1046/j.1460-9584.... |
