Even though I took a bit from both answers in my translation, neither of them seemed quite right - thanks anyway for all your help!
Non-ProZ.com Oct 29, 2004:
I've been thinking about this a bit more - could it be acknowledges and places on record?

acknowledges and makes it being acknowledged

I think the verb to acknowledge makes here better sense than what the dictionary says (to notice, to observe, to establish)

fa constatare in my opinion involves the rest of the assembly (people), where the president tells them that everything is ok. To place on record would be a written activity of the president, therefore something different

agree Katherine Zei : acknowledges and places on record, not "makes it being". Ciao!

determined and put on the record

I believe you are transalting minutes from a meeting. This is how I would put it.

Constata = ascertain / determine;
fa constatare - literally "has the other persons present ascertain"
