This has been answered in the telecommunications field, but this usage is talking about a faulty product being missed and allowed to continue along the production process in a manufacturing plant/factory to another process/line or continue on to be shipped out as a finished product (containing the faulty component).
I know what it means, but have been unable to locate an appropriate term in English.

Both above answers also explain the meaning of the term, but if you are looking for one word to express 「不具合品の流出」, then "defect outflow" is commonly used in manufacturing facilities. "Defect release" and "defect runoff" are also used.

Thanks. I was going to go with "product mistakenly slipping/getting/passing through", but the register of this is better. I am always a little wary of using bilingual documents originating in Japan as a reference, as one is never certain of who produced the English, but your support of this as correct alleviates those concerns. 流出 usually is defined as flow out, flow on, discharge, etc.
In your case, it seems that faulty product mistakenly slipped through the production line.

run off, or runoff (as a noun)
”run off” by not going through a proper process in the production line as in slipping through a process or skipping it. You can use this as a noun by connecting these two words and say "runoff" as in 流出損失(=runoff losses)
