W skład zespołu orzekającego wchodzą: dyrektor poradni psychologiczno−pedagogicznej (bądź inna upoważniona przez niego osoba), psycholog, pedagog, logopeda, lekarz oraz inni specjaliści spoza poradni, jeżeli ich udział w pracach zespołu jest niezbędny.
The multidisciplinary team carrying out the evaluation consists of the director of the counselling centre (or another person appointed to preside over the team); a psychologist; an educator; a speech therapist; a doctor; and any other outside specialist whose participation the team considers essential.
