MRI diagnosing of patients with neurological symptomatology characteristic of carpal tunnel syndrome.
Material and Methods:
Sixteen patients with neurological deficit of n. medianus and n. ulnaris were examined. MAGNETOM EXPERT 1T SIEMENS apparatus was used with loop, T1W and T2W sequences in two planes, 3-5 mm section thickness.
In 5 patient paramagnetic contrast medium was injected intravenously.
MR angiography was performed in 7 patients.
Results:
MRI identified the cause of deficit in 10 out of 16 patients:
6 patients had cyst, tendon edema and perineural edema resulting from traumatic lesions of the wrist. In two cases tumors of skeletomuscular system were identified. There were also one case of wrist bone hyperplasia and one of fibrous thickening of retinaculum flexor. Seven of these patients were surgically treated (4 synovicial cyst, 2 tumors, 1 fibrous thickening of flexor retinaculum).
Conclusion:
MRI showed perfect correlation with surgical findings in all operated patients. MRI is the diagnostic method of choise in neurologically clearly defined carpal tunnel syndrome where organic lesion is suspected.
Traduzione - Italiano Scopo:
Utilizzo della risonanza magnetica per la diagnostica medica su pazienti con sintomatologia neurologica caratteristica della sindrome del tunnel carpale.
Materiale e Metodi:
Sedici pazienti con deficit neurologici del nervo mediano e del nervo ulnare sono stati esaminati.
E stata impiegata una struttura ad anello con un magnete da 1,0 tesla (modello MAGNETOM EXPERT), immagini pesate in T1 e T2 su due piani e spessore di fetta di 3-5 mm.
In 5 pazienti è stato iniettato il mezzo di contrasto paramagnetico per via endovenosa.
L'angiografia con MR è stata praticata a 7 pazienti.
Risultati:
La risonanza magnetica tomografica ha identificato le cause del deficit in 10 pazienti su 16:
6 pazienti riportavano delle cisti, edema al tendine ed edema perineurale causate da lesioni traumatiche del polso. In due casi sono stati rilevati tumori all'apparato scheletrico e muscolare. E' stato riscontrato anche un caso di iperplasia ossea del polso e un caso di ispessimento fibroso del retinacolo dei flessori. 7 di questi pazienti sono stati trattati chirurgicamente ( 4 cisti sinoviali, 2 tumori e un ispessimento fibroso del retinacolo dei flessori.
Conclusione:
La risonanza magnetica ha evidenziato una perfetta correlazione con i risultati chirurgici in tutti i pazienti operati.
La risonanza magnetica è il metodo diagnostico di scelta nella sindrome del tunnel carpale ben definita dal punto di vista neurologico nel momento in cui si sospetta una lesione organica.
