Does this mean that he can be called a "grade 2 diagnostic radiographer"
(this leads me to wonder about the distinction, if there is one, between "diagnostic radiographer" and "diagnostic radiologist")

or does it refer to the type of radiodiagnostics / diagnostic radiology?

Based on the Wikipedia article: https://cs.wikipedia.org/wiki/Radiologie, I believe that the talk is about the so-called 2nd attestation of medical doctors in radiology (1995), while there is a single attestation nowadays (plus potentially a specialized attestation).

I don't want it to look as if "grade 2" refers to the level of his qualification, if this isn't correct.
From the wording, it looks as if "druhého stupně" refers to the field he specialises in, "radiodiagnostika". Perhaps "second level" or "level 2" sounds better.

The wording is arbitrary, but to your question - physicians need to take 2 levels of exams/certification, a general one, Level 1, and a specialist one, Level 2. See the link for more.
