: An English speaker would refer to 'a quarter' rather than 'a fourth'.

Yes, in Am. En."a fourth" is quite common. Both are correct, anyway. Thank you.

Quarter/Quarter/Half
I think that what is meant in the section you are querying is that the person has to take the tablet in this order: one quarter, one quarter, then half.
This drug certainly comes quarter-scored/cross-scored to make it easier to take quarters or halves. See:
•Rivotril 2 mg Tablets are cross scored so that they can easily be broken into halves or quarters. This can allow you to take lower doses at the start of your treatment.

This is a bit of a guess (albeit an educated one) as I'm not usually a medical translator. I just think that I would read it like this.

: it is not modified according to how the patient feels. The dosage is referred to meals//I work in a hospital. We refer dosage to meals (time).

Where it says 'de encontrarse nerviosa/relajada/ansiosa', the patient can increase or reduce the dosage, it is according to how the patient feels. There is no mention of meals in the text.

Por eso, aunque no se indique nada en el original, es un "acuerdo" internacional para indicar las dosis, en relación a las comidas principales. Saludos.
