Is there an exact or some neat way to say this in English? It's about some the results of some tests which were carried out on behalf of a patient who may have been in this condition at the time
La dimissione protetta consiste nella presa in carico del paziente adulto e anziano che, concluso l’iter diagnostico, curativo e terapeutico ospedaliero, ha perso temporaneamente o stabilmente l’autonomia e richiede pertanto interventi di carattere sanitario e/o sociale effettuabili a domicilio o nelle altre strutture territoriali. La dimissione protetta deve assicurare la continuità assistenziale fra ospedale e territorio.
La dimissione protetta consiste nella presa in carico del paziente adulto e anziano che, concluso l’iter diagnostico, curativo e terapeutico ospedaliero, ha perso temporaneamente o stabilmente l’autonomia e richiede pertanto interventi di carattere sanitario e/o sociale effettuabili a domicilio o nelle altre strutture territoriali. qui parlano di early supported discharge, non so se sia necessario "early" nel tuo caso.
CONCLUSIONS:: early supported discharge provided a cost-effective alternative in the management of stroke care. However, a larger study is required to assess the generalisability of the results and long-term cost effectiveness

There are many references on the Internet to this procedure. Sometimes the aftercare staff meets with the patient before release.
