Would it be correct to translate it:
Mother's maiden name
or
Father's mother family name.
What is customary? When translating official documents we go word by word or the way it makes sense?

Thank you all for your help. I will keep the original as Father's mother family name. It doesn't sound right but I think AlisaIWW as a very good point here.

Going word-for-word would be too complicated. We know that the meaning is "mother's maiden name" so I would just simplify it instead of saying "mother's father's last name".

I would go word-for-word, since 'mother's maiden name' does make sense, but only most of the times, not always: it could be the case that the mother was given/took a last name different from that of her father. And after all, they didn't ask about שם נעורים של האם, which they certainly could: http://www.google.com/search?hl=en&q="שם נעורים של האם"&aq=f...

Maternal grandfather's surname

...which may be different to your mother's maiden name. It is not asking for שם נעורים של האם.
