buona detersione della regione basale, ove resiuda discreto **affastellamento strutturale **, e del versamento pleurico omolaterale. Ridotta la velatura pleurica segnalata a sinistra.
I just can't come up with an appropriate, corresponding term in English - it would seem to be saying that the 'right basal region of the lung is largely clear but moderate *** remains, and some ipsilateral pleural effusion.'
Any suggestions would be appreciated. I think I understand the general meaning but just can't hit on the form of words!

"Increased markings" should work o.k. here. It usually comes with a qualifier in English reports ("bronchvascular markings", "interstitial markings"), but can be used without. I am not sure if the meaning of "affastellamento strutturale" includes such qualifier (and if so, which one), but I don't think so.

NB: "discreto" is less than moderate, it is more like very mild, or marginal.
