El SUNCT (short lasting unilateral neuralgiform headache attacks with conjuntival injection and tearing) es una cefalea primaria que se encuentra en el grupo 3 de la clasificación de la Inter¬national Headache Society1 junto con la cefalea en racimo y la hemicrania paroxística, denominadas en conjunto cefaleas autonómico-trigeminales. Este grupo comparte rasgos típicos como son la unilateralidad estricta del dolor, el lagrimeo, rino¬rrea o congestión nasal y ptosis palpebral con pupila miótica. El SUNCT fue descrito por Sjaastad y sus colaboradores en 1978 y posteriormente presentado como tal en 19892. Dada la baja frecuencia de presentación, muchos especialistas no han tenido la oportunidad (o suerte) de ver uno. Dentro del SUNCT se considera un subgrupo que se presenta clínicamente sin inyección conjuntival y/o lagrimeo denominado SUNA (short lasting unilateral neuralgiform headache attacks with cranial auto¬nomic features). El grupo de cefaleas autonómico-trigeminales se diferencia sobre todo por la duración de los episodios de dolor, con frecuencias decrecientes entre cefaleas en racimo, hemicrania paroxística y SUNCT3,4.
Las características clínicas del SUNCT son principalmente los ataques breves, que pueden oscilar entre 5 a 240 s de dura¬ción, con un promedio de 10 a 60 s. Lo breve del dolor favorece su alta frecuencia de presentación durante el día, con un pro¬medio de 30 crisis diarias. Las descripciones refieren entre 1 a más de 80 crisis por día. Caracteriza a este dolor la estricta unilateralidad, la intensidad, que siempre es moderadamente severa a muy severa, y la localización en la región periorbita¬ria y temporal, si bien puede además irradiarse a zonas veci¬nas del cráneo (nariz, mejilla, oído, cara). La descripción del dolor responde a descriptores neuropáticos: excrusiante, que¬mante, eléctrico, desgarrante. La presentación temporal del dolor se caracteriza por el corto tiempo de llegada a la máxima intensidad y el retiro brusco del mismo. Dentro de la crisis se describen tres tipos de presentación (fig. 1). No suele presentarse durante el sueño. En la evolución temporal de la presentación de los ataques hay periodos de remisiones y recaídas con duración variable de días a semanas 4-8.
Las manifestaciones constantes asociadas al dolor son la inyección conjuntiva y lagrimeo ipsilaterales. Los síntomas menos frecuentes de presentación son rinorrea, obstrucción nasal, edema palpebral, ptosis. Se han descrito también enro¬jecimiento facial, fotofobia y blefarospasmo. Si bien el dolor se presenta espontáneamente, hay descripciones de gatillos como masticar, abrir la boca, maniobra de Valsalva, estímulos táctiles en la zona y movimientos del cuello 4,7,8.

SUNCT (short lasting unilateral neuralgiform headache attacks with conjunctival injection and tearing) is a primary cephalalgia included within Group 3 of the International Headache Society1 classification together with cluster headaches and paroxysmal hemicranias, and jointly known as trigeminal autonomic cephalalgias. This group shares typical features such as strictly unilateral pain, lacrimation, rhinorrhoea or nasal congestion and palpebral ptosis with miotic pupil. SUNCT was described by Sjaastad et al. in 1978 and subsequently presented as such in 19892. In view of its low occurrence frequency, many specialists have not had the chance (or the good fortune) of seeing one. Within SUNCT there is a subgroup, that clinically shows no occurrence of conjunctival injection and/or lacrimation, called SUNA (short lasting unilateral neuralgiform headache attacks with cranial auto¬nomic features). The group of trigeminal autonomic cephalalgias differs mainly in the duration of pain episodes, with frequencies decreasing between cluster headaches, paroxysmal hemicranias and SUNCT3,4.
SUNCT is mainly characterized by brief attacks lasting from 5 to 240 s, with an average of 10 to 60 s. The briefness of pain favours a high frequency of occurrence throughout the day, with an average of 30 attacks daily. Descriptions report from 1 to more than 80 attacks daily. Characterization of pain is strictly unilateral, intense (which is always from moderately to highly severe), and localized in the periorbital and temporal region, although it may also extend towards neighbouring cranial structures (nose, cheeks, ears, face). The description of pain corresponds to neuropathic features: stabbing, burning, electric shock-like and pricking. Onset of pain is characterized by a brief time to maximum intensity and sudden remission. There are three types of attacks (Figure 1). Generally, these do not occur during sleep. The time course of attack occurrence shows periods of remissions and relapses varying in duration from days to weeks.4-8
Events that invariably accompany pain are ipsilateral conjunctive injection and tearing. Other less frequent symptoms include rhinorrhoea, nasal blockage, palpebral oedema and ptosis. Reddening of facial area, photophobia and bleparospasm have also been reported. Although pain appears spontaneously, triggers have been reported, such as chewing, opening the mouth, the Valsalva manoeuvre, cutaneous stimuli in the area, and neck movements 4,7,8.
