1. Introduction
You are invited to take part in a research study, referred to as ABC-123, which is organized and funded by Pharma Company.
The information contained within this Patient Information and Informed Consent Document is very important; it tells you about what the study involves. Please make sure that you read it carefully and understand it. If you have any questions, the study doctor will try to answer them. Once you have read this information and if you agree to take part, you will be asked to sign this consent document. This document says that you have been informed about the study and give permission to take part in the study.
Study Assessments
Blood sampling
Blood samples will be collected at all visits. One sample at each visit will be for routine safety analysis (about 13.5 ml (about 3 teaspoons) each time) and on 3 occasions (spread across Visits 3, 4, and 5) an additional sample of 5 ml (1 teaspoon) will be taken for measurement of the amount of study medication in your blood. The timing of these samples will be arranged with your study doctor.
The maximum amount of blood to be taken over the study for any patient will be approximately 111 ml (this is about 7 table spoons).
Urine sampling
Your urine will be collected and tested at all visits. If you have symptoms of a urinary tract infection (UTI), a urine culture will be conducted and this will be part of the study procedures and you will not have to pay for this test. If a UTI is found at screening, you will need to be treated for it before you can participate in the study.
INFORMED CONSENT
I freely and voluntarily consent to participate in the study and I authorize the release of my medical records, for research or regulatory purposes to the sponsors, the Contract Research Organization, Institution Review boards and study doctors. I also understand that I shall receive a signed copy of this document.

Avaliações do Estudo
Amostras de sangue
As amostras de sangue serão coletadas em todas as visitas. Uma amostra em cada visita será coletada para análise de segurança de rotina (aproximadamente 13,5 mL ou 1 colher de sopa), e em 3 ocasiões (durante as Visitas 3, 4 e 5) uma amostra adicional de 5 mL (aproximadamente uma colher de chá) será coletada para medir a quantidade de medicação do estudo em seu sangue. As coletas destas amostras serão organizadas com o médico do estudo. A quantidade máxima de sangue coletada durante o estudo será aproximadamente 111 mL (cerca de 7 colheres de sopa).
Amostras de urina
Sua urina será coletada e testada em todas as visitas. Se você apresentar sintomas de infecção do trato urinário (ITU), uma cultura de urina será realizada. Se a ITU for identificada durante a triagem, esta deverá de ser tratada antes que você possa participar no estudo.
DOCUMENTAÇÃO DE CONSENTIMENTO

Source text - Portuguese 3. PREÇO E REAJUSTE/CONDIÇÕES DE PAGAMENTO:
Os preços são os constantes do Pedido de Compra e Planilhas de preços que integram o presente Contrato. Os preços serão fixos durante a vigência do Contrato. A data de pagamento está estabelecida em outra parte deste Contrato, ou se não estiver determinada, deverá ser 47/semanal ("47/weekly"), que prevê que o pagamento deverá ser feito em média em 47 dias, com desembolsos semanais subsequentes ao recebimento das mercadorias na planta da Compradora, ou, se for instruído pela Compradora, ao recebimento de uma fatura válida.
3.1 - Os preços foram compostos considerando-se todos e quaisquer custos, diretos e indiretos, que a Vendedora venha a incorrer para a execução dos Serviços e/ou fornecimento das Mercadorias, incluindo materiais de consumo indireto, perdas de material, impostos, taxas, seguros, honorários, lucros, despesas e encargos com mão de obra.
3.2 - O preço será pago pela Compradora à Vendedora, mediante a realização de inspeção dos Serviços e/ou das Mercadorias e com a constatação de que foram completamente executados e/ou entregues. Não serão pagos os Serviços ou as Mercadorias recusados pela Compradora, ou aqueles que deverão ser refeitos e/ou substituídos pela Vendedora, até que definitivamente aceitos. Constatada a não execução completa dos Serviços ou a não entrega integral das Mercadorias, o pagamento será suspenso até a efetiva conclusão dos Serviços ou a entrega integral das Mercadorias, e até que nova inspeção seja realizada nos termos deste Contrato.
3.3 - A Vendedora emitirá a Nota Fiscal relativa aos Serviços executados e aceitos e/ou às Mercadorias fornecidas e aceitas, e a entregará à Compradora no prazo previamente estabelecido. Eventuais valores cobrados por Serviços e/ou Mercadorias não aceitas pela Compradora e/ou não executados ou não entregues pela Vendedora serão deduzidos do valor a ser pago.
3.4 - Na execução de Serviços que requeiram o uso de materiais, serão emitidas Notas Fiscais relativas aos materiais efetivamente utilizados se não tiverem sido fornecidos pela Compradora e desde que tenham sido por ela solicitados. A Vendedora manterá pelo prazo mínimo de 6 (seis) meses, as Notas Fiscais dos materiais que adquirir para a execução dos Serviços, comprometendo-se a apresentá-las à Compradora sempre que solicitado.
3.5 - Sempre que solicitado pela Compradora, a Vendedora emitirá relatórios mensais de despesas, identificados por local de execução dos Serviços, dos quais constarão: (a) identificação das Ordens de Serviço; (b) descrição de cada item de material, quantidades utilizadas, valores unitários e totais; (c) descrição da mão de obra utilizada, e respectivos valores envolvidos; e (d) número do "kardex" dos materiais utilizados e dos Serviços executados.
3.6 - A Vendedora obriga-se a cumprir rigorosamente as obrigações tributárias principais ou acessórias previstas na legislação vigente, que incidam ou venham a incidir sobre os Serviços ou Mercadorias objeto deste Contrato, recolhendo todos os tributos incidentes nos prazos e nas formas estabelecidas pela legislação pertinente. A Vendedora entregará mensalmente à Compradora, juntamente com a Nota Fiscal, os comprovantes de recolhimento no prazo legal dos tributos incidentes sobre os Serviços e/ou Mercadorias ora contratados.
3.7 - A Vendedora obriga-se, por si e por seus subcontratados, a cumprir rigorosa e pontualmente as obrigações trabalhistas e previdenciárias previstas nas cláusulas 12.2 e 12.3 deste instrumento em relação aos empregados destacados para a execução dos Serviços ("Empregados Autorizados"). A Vendedora entregará mensalmente à Compradora, juntamente com a Nota Fiscal, os comprovantes de recolhimento no prazo legal dos encargos trabalhistas, previdenciários, acidentários, securitários e tributários relativos aos Empregados Autorizados.
3.8 - Não sendo apresentados os comprovantes dos recolhimentos de que tratam as cláusulas 3.6 e 3.7, a Compradora poderá adotar uma das seguintes medidas, a seu exclusivo critério:
(a) reter o pagamento devido pela prestação dos Serviços ou pelo fornecimento das Mercadorias até que a Vendedora apresente tais comprovantes;
(b) reter o pagamento devido pela prestação dos Serviços ou pelo fornecimento das Mercadorias, destinando-os, por conta e ordem da Vendedora, à realização dos recolhimentos aqui previstos que estiverem em atraso. Se solicitado pela Compradora, a Vendedora emitirá imediatamente as necessárias guias para a realização de tais recolhimentos. Eventuais créditos remanescentes em favor da Vendedora, após a realização dos recolhimentos em atraso na forma aqui estabelecida, serão pagos pela Compradora à Vendedora, dando-se por quitada integralmente a respectiva obrigação de pagar; ou
(c) considerar imediatamente extinto o presente Contrato na forma prevista na cláusula 8.
3.9 - A Vendedora se obriga a entregar para a Compradora, na periodicidade por ela indicada, as certidões negativas de débitos tributários e previdênciários devidamente atualizadas, tais como, mas não se limitando, a Certidão Negativa de Débito do INSS, Certificado de Regularidade do FGTS, Certidão Negativa de Débito da Receita Federal e da Procuradoria da Fazenda Nacional e Certidão Negativa de Débito do ISS.
3.10 - Nos casos de fornecimento de ferramental, a Nota Fiscal deverá ser apresentada pela Vendedora à Compradora somente após a aprovação pela Compradora de toda a amostra de produção e inclusão pela Vendedora de informações sobre o ferramental no módulo de gerenciamento de ferramentas localizado no Sistema Global de Monitoração de Qualidade (GQTS - Global Quality Tracking System - Tool Management). A Nota Fiscal será enviada pela Vendedora à Compradora acompanhada dos seguintes documentos: (i) cópia assinada do Contrato de Comodato - Máquinas e Ferramentais e (ii) cópia do(s) relatório(s) de inspeção de peças de teste aprovadas.
Translation - English 3. PRICE AND ADJUSTMENT/PAYMENT TERMS:
The prices are those contained in the Purchase Order and in the Price Charts making an integral part hereof. The prices shall be fixed during the validity hereof. The payment date is set forth elsewhere in this Contract, or if not stated, shall be 47/weekly, which provides that payment shall be made on average in 47 days, with weekly disbursements, following receipt of the goods at Buyer's plant or, if directed by Buyer, receipt of a valid invoice.
3.1. The prices were made up considering any and all direct and indirect costs that Seller may incur to perform the Services and/or to provide the Goods, including indirect consumption materials, losses of material, taxes, duties, insurances, fees, profits, expenses and charges connected with labor.
3.2. The price shall be paid by Buyer to Seller in conducting the inspection of the Services and/or of the Goods, and in ascertaining that they were completely performed and/or delivered. The Services or the Goods rejected by Buyer, or those that Seller shall redo and/or replace, shall not be paid until they are definitively accepted. After it was ascertained that the Services were not completely performed or that the Goods were not fully delivered, the payment shall be suspended until the Services are effectively concluded or the Goods are fully delivered, and until another inspection is conducted hereunder.
3.3. Seller shall issue the Invoice referring to the Services performed and accepted and/or to the Goods provided and accepted, and shall deliver it to Buyer within the term previously established. Contingent amounts charged on Services and/or Goods that were not accepted by Buyer and/or were not performed or delivered by Seller shall be deducted from the amount to be paid.
3.4. Upon the performance of Services requiring the use of materials, Invoices referring to the materials effectively used shall be issued, if Buyer did not provide them, and provided that said materials have been requested by Buyer. Seller shall keep for a term no shorter than six (6) months, the Invoices of the materials Seller acquires for the performance of Services, undertaking to present them to Buyer, whenever requested.
3.5. Whenever requested by Buyer, Seller shall issue monthly reports of expenses identified by the place where the Services were performed, which will contain: (a) identification of the Service Orders; (b) description of each item of material, quantities used, unit and total values; (c) description of the labor used, and respective amounts involved; and (d) "Kardex" number of the materials used and of the Services performed.
3.6. Seller undertakes to rigorously fulfill the main or accessory tax obligations provided for in the prevailing laws and regulations, which are imposed or may be imposed on Services or Goods that are the purpose of this Contract, paying all the taxes imposed within the terms, and as established in the proper laws and regulations. Seller shall monthly deliver, together with the Invoice, the vouchers of payment within the legal term of the taxes levied on the Services and/or Goods herein contracted, to Buyer.
3.7. Seller undertakes, by itself and its sub-sellers, to rigorously and punctually fulfill social security and labor obligations provided for in clauses 12.2 and 12.3 hereof in relation to the employees allocated to perform the Services ("Authorized Employees"). Seller shall monthly deliver, together with the Invoice, the vouchers of payment, within the legal term, of labor, social security, work-related accident, insurance-related and tax charges referring to the Authorized Employees, to Buyer.
3.8. In case the vouchers of payment referred to in clauses 3.6 and 3.7 are not presented, Buyer may adopt, at its own discretion, one of the following measures:
(a) To withhold the payment due to the performance of Services or to the provision of Goods until Seller presents such vouchers;
(b) To withhold the payment due to the performance of Services or the provision of Goods allocating them, by order and on account of Seller, to the payments herein provided for that are late. If requested by Buyer, Seller shall immediately issue the necessary forms to make such payments. The potential remaining credits for Seller, after the late payments are made, as herein established, shall be paid by Buyer to Seller, the respective obligation to pay being fully discharged; or
(c) To consider this Contract immediately extinguished, as provided for in clause 8.
3.9. Seller undertakes to deliver, in the periodicity designated by Buyer, the debt clearance certificates regarding taxes and social security contributions, duly updated, including, but not limited to Debt Clearance Certificate by the INSS ("Brazilian
Social Security Institute"), Certificate of Good Standing with the FGTS ("Workers' Severance Pay Fund"), Debt Clearance Certificate by the Federal Revenue Department and the Attorney General's Office of the National Treasury and ISS ("Tax on Services") Debt Clearance Certificate, to Buyer.
3.10 - In case of provision of tooling, the invoice shall be submitted by Seller to Buyer only after full production sample is approved by Buyer and input of tooling information into the tool management module located in GQTS - Global Quality Tracking System - Tool Management, by Seller. The invoice shall be sent by Seller to Buyer accompanied by the following documents: (i) signed copy of the Free Lease Agreement - Machinery and Tooling and (ii) copy of approved sample part inspection report(s).
