Translation - English Chronic Obstructive Pulmonary Disease (COPD) is a preventable disease state characterized by limitations in lung airflow that are not fully reversible. Besides having impact on lungs COPD might also cause severe systemic effects1. Cigarette smoking is the most important risk factor for COPD development and responsible for 80 to 90% of cases 3, 4. In comparison to other people, increased number of CD+8 T lymphocytes are found in the airways of smokers who developed COPD 5. Recent studies suggested significant evidence highlighting the contribution of acquired immunity to COPD pathogenesis. Dysfunctions in acquired immune response may occur due to a defect in regulation of immune cells (T and B lymphocytes) or due to cell death and also as a result of production of (self or foreign) new antigens. Cigarette smoking, primary risk factor for COPD may change proliferation and death pathways of lymphocytes and may lead formation of new antigenic epitopes through direct oxidation of emerging proteins.

Translation - English β Thalassemia is a hereditary hemoglobin disorder with an autosomal recessive transmission caused by the reduced synthesis of β globin chain which incorporates into the hemoglobin structure. Over 200 mutations and deletions on the β globin gene may lead to various degrees of reduction in the synthesis of β globin chain. In case a mutation or deletion exists on both copies of the β globin gene pair each one of which is inherited from one of the parents then the insufficiency of β globin chain production results in an excess amount of α chains which are awaiting to assembly into a hemoglobin tetramer. Hemes and hemichromes generated as a result of the oxidation of such free α chains attach to the erythrocyte membrane leading to the impairment of membrane structure. Erythrocytes with degraded membranes are subject to ineffective erythropoiesis in bone marrow and hemolysis within the peripheral blood. This situation in turn gives rise to deep anemia and erythroid hyperplasia and enlargement of bone marrow. In association with the severity of the inherited genetic defect (genotype), clinical severity (phenotype) of homozygous β Thalassemia has a clinical spectrum ranging from severe transfusion-dependent anemia (Thalassemia major) to severe anemia which, however, allows development and thrive without transfusion (Thalassemia Intermedia). Almost 85% of the all Thalassemia major cases are lost due to anemia at the age of around 5 unless they are regularly transfused with blood.

An adverse event (AE) is any untoward medical occurrence (i.e., any unfavorable and unintended sign [including abnormal laboratory findings], symptom or disease) in a subject or clinical investigation subject after providing written informed consent for participation in the study. Therefore, an AE may or may not be temporally or causally associated with the use of a medicinal (investigational) product.
The occurrence of adverse events should be sought by non-directive questioning of the patient at each visit during the study. Adverse events also may be detected when they are volunteered by the patient during or between visits or through physical examination, laboratory test, or other assessments.
Abnormal laboratory values or test results constitute adverse events only if they fulfill at least one of the following criteria:
• they induce clinical signs or symptoms,
• they are considered clinically significant,
• they require therapy.

MADDE 1- (1) Bu Yönergenin amacı; evde sağlık hizmeti sunumuna ihtiyacı olan bireylerin muayene, tetkik, tahlil, tedavi, tıbbi bakım ve rehabilitasyonlarının evinde ve aile ortamında sağlanması, bu kişilere ve aile bireylerine sosyal ve psikolojik destek hizmetlerinin bir bütün olarak birlikte verilmesi için Sağlık Bakanlığına bağlı sağlık kurumları bünyesinde evde sağlık hizmetleri birimleri kurulması, bu birimlerin asgari fiziki donanımı ile araç, gereç ve personel standardının ve ilgili personelin görev yetki ve sorumluluklarının belirlenmesi, iletişim, uygulanacak randevu, kayıt ve takip sisteminin tanımlanması ve uygulamanın denetimine ilişkin usul ve esasların belirlenerek evde sağlık hizmetlerinin sosyal devlet anlayışı ile etkin ve ulaşılabilir bir şekilde uygulanmasını sağlamaktır.
Kapsam
MADDE 2- (1) Bu Yönerge, Sağlık Bakanlığına bağlı olarak faaliyet gösteren ve Sağlık Bakanlığınca, bünyesinde evde sağlık hizmetleri birimi kurmak suretiyle bu hizmetleri vermeye yetkili kılınacak yataklı tedavi kurumlarını, ağız ve diş sağlığı merkezi ve hastanelerini, toplum sağlığı merkezlerini ve buralarda çalışan personel ile aile hekimlerini, aile sağlığı merkezlerini ve aile sağlığı elemanlarını kapsar.

SECTION ONE
Objective, Scope, Legal Basis and Definitions
Objective
ARTICLE 1- (1) The purpose of this Directive is to identify the procedures and principles pertaining to the provision of examination, investigation, testing, treatment, medical care and rehabilitation in household and family environment to the individuals who need to be offered home care service; pertaining to the establishment of home care service units within the body of the healthcare institutions under the Ministry of Health so that social and psychological support services can be offered as a whole to such people and their families; pertaining to the determination of the minimum physical outfit as well as the equipment, device and staff standards and the duties, authorities and responsibilities of the relevant staff; pertaining to the communication; to the description of the appointment, recording and follow-up system to be applied; and also to the supervision of the implementation and thus to ensure an effective and accessible implementation of home care services.
Scope
ARTICLE 2- (1) This Directive covers the inpatient treatment facilities, oral and dental health centers and hospitals, public health centers which operate under the Ministry of Health and will be authorized by the Ministry of Health to provide the home care services through establishment of home care service units within their bodies as well as the staff employed therein in addition to family physicians, family health centers and family health caregivers.
Legal Basis
ARTICLE 3- (1) This Directive has been drawn up based on the articles 3 and 9 of Fundamental Law number 3359 dated May 7, 1987 on Healthcare Services and the article nr. 43 of the Decree Law no. 181, dated Dec 13, 1983 on the Organization and Duties of the Ministry of Health.
