Disease still not fully understood

The 1992 movie "Lorenzo's Oil" brought a rare disorder called adrenoleukodystrophy (ALD) to the world's attention. It is a progressive degenerative myelin disorder, meaning that myelin, the "insulation" around nerves, breaks down over time. Without myelin, nerves can't function normally, or at all. Unfortunately, the body can't grow replacement myelin, so the disorder is progressive -- it gets worse over time.
What Causes ALD, and Who Develops It?
ALD is an inherited recessive genetic disorder linked to the X chromosome. Because of the way genetic inheritance works, only boys have the most severe form of ALD. The disorder leaves the body unable to break down big fat molecules, either ones the body makes itself or ones that enter the body through food. Research has shown that this is most likely due to a carrier protein that fails to work correctly and carry the fat molecules to where they would be broken down. The fat molecules build up and clog up cells, and hurt nerve cells in the brain and spinal cord. ALD affects about one in 20,000 males.
One gene, called ABCD1, has been identified as being associated with ALD. Genetic testing can be done to see if a woman has the defective gene. This way, a woman who may have inherited an abnormal gene will know for certain whether or not she has it (is a carrier) and could pass it on to her children.

En 1992 la película “Un milagro para Lorenzo” atrajo la atención del mundo por una rara enfermedad llamada adrenoleucodistrofia. Es una enfermedad progresiva degenerativa de la mielina, esto quiere decir que la mielina, el “aislamiento” alrededor de los nervios, se estropean con el tiempo. Sin mielina, los nervios no pueden funcionar normalmente o del todo. Desafortunadamente, el cuerpo no puede generar un remplaza de ésta, por lo tanto la enfermedad es progresiva – empeora con el tiempo.
¿Qué causa el ALD y quién lo desarrolla?
El ALD es una enfermedad genética recesiva hereditaria ligada al cromosoma X. Debido a la forma en que trabaja la genética hereditaria, sólo el sexo masculino posee la forma de ALD más severa. La enfermedad deja al cuerpo incapacitado para disolver las grandes moléculas grasa, ya aquellas que produce el mismo cuerpo o que ingresan a través de la comida. La investigación ha demostrado que esto es más probable debido que una proteína portadora falla en trabajar correctamente y transportar las moléculas grasas a donde se disolverían. Las moléculas grasas bloquean y obstruyen células, y dañan las células nerviosas en el cerebro y la medula espinal. El ALD afecta a uno de 20.000 hombres.
Un gen, llamado ABCD1, ha sido identificado al estar asociado con el ALD. Un examen genético podría comprobar si una mujer tiene el gen defectuoso. De esta manera, una mujer que puede haber heredado un gen anormal sabrá con certeza si ella es portadora o no y si podría transmitirlo a sus hijos.
