Non evidenti rime di frattura.
I'm presuming SN is for sinistra - so it's the left shoulder - and perhaps int. is for interno and est. esterno? - E for 'and' - but what about the R! I wondered if the R could stand for rotto, but this seems unlikely given the next line and the patient is just suffering from fibromyalgia.
If anyone could help I'd be so grateful!

I agree with SN = sinistro, so I left it out of the answer I suggested. It would be better to be able to see the complete text of the sentence before the "SN".

Clearly the exact meaning of R. is ambiguous here. I'm still inclined toward "rotatore" -- although not 100% certain, because it seems less likely that the X-ray would be reporting anything about the movement (rotation) of the shoulder. And ... it's still also possible that there might be another meaning altogether!

Asker: Thanks, that seems like a possibility. the previous sentence wouldn't help as is concerned the heart. 'Immagine caridaca a salienza ventricolare sinistra'

Asker: Actually, I now think it's most likely to be be rotation rather than rotators - see the links I've posted as a discussion entry which another ProA member sent me. But thanks for your input Joseph. Hope you find the links useful too. I'm still regularly referring to the acronym list you kindly supplied me with some time ago!
