The patient (who suffers from depression) is reported a being 'vigile, lucida, collaborante, orientata nei quattro domini.'
Seems along the lines of 'firing on all four cylinders' but I'm struggling to think of something less colloquial.

This is a standard, widely used expression in mental health evaluations. (It does mean "firing on all 4 cylinders" ... when you understand the nature of each cylinder.)

I would think you could also say "oriented in all 4 spheres" although I find very few instances of this phrase in an online search.
